The Root Cause Analysis Steps
A root cause analysis (RCA) offers one of the methods to solve problems by understanding the initial cause. The types of RCA differ and include physical causes, from a tangible item, human causes, often human error or negligence, and organizational causes, often due to system or process failure.
Depending on the industry, the RCA team typically consists of a subject matter expert, a process supervisor or manager, a quality specialist, and a process improvement specialist. Across the board, however, an RCA follows five distinct steps: problem identification, data collection, causative factor identification, root cause determination, and recommendation.
